Change of Payment Form
All changes to payment terms must be made at least 10 business days prior to the monthly payment date. If your monthly payment is scheduled within the next 10 business days, you are responsible for making payment. Payment is non-refundable. Completion of this form modifies the payment terms of your existing Enrollment Agreement with Devita Karate. All other terms of said agreement are valid and enforceable.
